This has happened to me on XCOM EW, but never in Silent Storm. Apparently 100% isn't 100%. Luckily it didn't happen against an enemy or in a tough spot. (As I'm doing an impossible ironman run atm, I can't reload if BS like this happen :P )

 

    Yeah that happens when you are taking a point blank shot with a rifle. For some reason it zips over the head usually. I think kneeling might negate that or backing off by a tile or two. ;)
